Health Intervention Program for Families - HIP

Purpose:
The HIP team provides services to children with health problems
and their at-risk families. Services such as family and environmental
home assessment, educational visits and referrals to community agencies
are tailored to the needs of each family. Families are encouraged
and assisted to use services already in place as well as discover
new resources.
Staff:
Care is delivered by public health nurses, community health paraprofessionals,
social workers and counselors. A substance abuse counselor provides individual,
at-home health education, counseling, and referral services for
program participants.
Referrals:
Family members who would like such a service are welcome to phone.
Community services are welcome to make referrals to this program.
Blank referral forms are available upon request. Referrals are
accepted by phone, fax, or mail by any interested person on behalf
of the family who would be involved.
