This unit is supervised by the Senior Associate Dean of Students. The members of the unit promote and support high standards for conduct in our community, supervise residential life, identify and mentor student leaders, encourage and support involvement in student organizations, train and develop our staff, and provide programs of welcome and transition for new students.


Incident Report

  • To use the electronic version of the Incident Report, right-click on "Incident Report" above, and click "Save Target As..."
  • Save the Incident Report template to an appropriate location on your personal computer.
  • Open the Incident Report template from where you saved it on your computer, and a series of Question Boxes will prompt you to enter certain information.
  • Be clear; be descriptive; be appropriate.
  • Try to form an objective perspective based on facts.
  • Keep in mind that an incident report is a confidential document, and the only individuals who will see the document are administrators and certain relevant parties, who may call you to follow up on the incident or to ask questions.
  • Fill out the remaining portions of the form. When completed, go to File, and click "Save As..." to save your Incident Report.
  • Save your report onto your computer. Access your La Salle E-mail through mylasalle, and send the saved Incident Report to badstubn@lasalle.edu.
