Clearance Forms

For all FOCUS volunteers:

Here are PDF copies of the clearance forms required for any volunteer or teaching programs. Feel free to download and fill out at your convenience. These forms MUST be filed within the first two weeks of your program, or you will not be able to volunteer.

Important Change for 2009:

UMAS will only be contributing HALF of the $20 clearance submission cost. Please have $10 in cash or check (made out to La Salle Ministry & Service, for: Clearances) with your completed forms when you drop them off at the chapel.

Also, all completed clearances will be mailed to the UMAS Chapel office. You will be notified through email when yours is ready for pick-up.

When you submit the forms, make sure your name is crossed off the FOCUS list. Thanks for helping and enjoy your volunteer experience!
