Feel free to download and electronically fill out the application forms. When finsihed, feel free to email all forms (except Recommendations*) to umas@lasalle.edu. All forms must be received or postmarked by January 15, 2009.
To edit the forms electronically, right-click the form, click "Save As..." and save the document as the student's name. Adobe Acrobat is required to save the filled in forms. |
