Horoscope Accuracy Survey

I am conducting an experiment to determine the accuracy of astrological personality profiles. This form acts as the front-end to a program that can (supposedly) describe your personality based on your birthdate.

Please fill in the requested information and then click "Submit". You will then be taken to another form that will show you the output of the program and ask you to rate the accuracy of the profile on a scale of 1-5, with 1 being least accurate and 5 being most accurate. When you get your results, please read through them and rate them immediately, based on your "gut" reaction. This should minimize potential bias in the statistics. Try not to be influenced by any preconceived notions you have regarding astrology and horoscopes.
