COVID-19 Vaccine requirement
La Salle requires that all members of the University’s campus community receive an initial COVID-19 vaccine:
- 2 doses of the Pfizer-BioNTech vaccine OR
- 2 doses of the Moderna vaccine OR
- 1 dose of the Johnson & Johnson/Jannsen vaccine
How to submit your vaccination or exemption request:
- Full-time, undergraduate students: Your COVID-19 vaccination is included in the Health History Form that you will submit to Student Health. You may request an exemption by submitting the Student COVID-19 Vaccination Exemption Request form..
- Part-time, undergraduate students: You will submit your COVID-19 vaccination through the Student COVID-19 Vaccination Form. You may request an exemption by submitting the
- Graduate students: You will submit your COVID-19 vaccination through the Student COVID-19 Vaccination Form. You may request an exemption by submitting the Student COVID-19 Vaccination Exemption Form.
- Faculty and Staff: You will submit your COVID-19 vaccination OR exemption request through the Employee COVID-19 Vaccine Status Certification Form.
Masks are available on campus
Masks are optional on campus, and required in certain areas such as the COVID-19 testing center, clinical settings, and some classrooms at faculty discretion. The following locations on campus have supplies of surgical and KN-95 masks available for free to all students, faculty and staff throughout the semester.

COVID-19 Dashboard
La Salle University’s COVID-19 dashboard captures the number of estimated confirmed cases of COVID-19 among employees, residential students, and non-residential students who maintain a campus presence. (You will receive notification from a representative of the University’s or City of Philadelphia’s contact tracing efforts if you have been in close contact with one of these cases.)
The dashboard will be updated at least once weekly.
